Hi Alex,

I just looked at the form myself and see exactly what you're saying. I didn't create the form, however. The form was given to us by the radio show for sponsorship purposes. With that being said, I will get in touch with the radio show people and find out if there is a way to adjust the form and if the form you have filled out and sent over is okay.

I think the best way to end the confusion is just to have you give me your email and the information and I'll fill out the form for you on my computer and send it back to you, unless you feel more comfortable filling it out yourself if you can. From there you can send it to sales@podfireradio.com if everything's okay.
