Karen He probably removed the bolt when he was running around in the train station on Grace's wild-goose chase.

I think he can see things that will happen, and he can see that if he does X, then Y will happen--but he can't necessarily see *why* Y will happen. So he knew that if he irritated his cousin with his watch, his cousin would die. But he didn't necessarily know that his cousin would throw the watch out the window, causing the workers to abandon their fish sandwiches, which caused the cat to jump on the wall which caused the wall to fall on his cousin.

The cousin doesn't seem like a great loss, but I hope the cat was able to jump clear.
