He’s definitely been buried alive. They did it while he was knocked out on sleeping pills which is why he didn’t wake up until he was in a coffin underground.
So if he was buried alive, what happened to sweet Columbo?
Sandy says to him at the end “You know, I always wanted a black Labrador. If you ever need someone to take care of him, I’m your woman” - so I think that’s the author’s way of saying Columbo will be fine. The women on the island will keep the dog.
