The big heart the doctor is talking about is part of congestive heart failure. The heart can no longer keep up with its workload, i.e. it can't pump enough blood for the body. The heart enlarges as it tries to compensate, but that just wears it out faster. It's a chronic condition.
I also agree with Leeann. And I admit that I hooted to when Parvana (don't know how to spell it...I listened to the audiobook) laughed in reaction to Ove's heart being too big!
