I wasn't sent a confirmation page. I was only sent one from my amazon account for the preorder.

My publisher said that you wouldn't be emailed a confirmation. After you filled out the form you would have been directed to a confirmation page stating that you had filled it out. Please let me know if you weren't directed to that page.
