I felt like with all the train references throughout, she stepped in front of the train. She was on a repetitive cycle, boarding the "train of life" and boarding the train to go to Voyager and to go home. Also, she was mentally fixated on the vision of people killing themselves that way somewhere in the beginning of the book, and "I have yet to hear about a woman on the tracks"
