I picked up on the Munchausen by proxy element as well, and wondered whether there was a possibility that the mother was somehow the cause. It was one of the many subtle subplots like the father being a 'fascist', the insinuation that Capaldi was potentially molesting Josie, the 'lifted' kids and so many other little almost throw away ideas.
