I thought the smile was mysterious because she knew how crazy it looked, both in a literal way (ew) and in considering her life. Most people don't believe in unconditional kindness. They wouldn't think someone so hurt and so close to death would just give away their last bit of strength for a stranger, yet she does it anyway.
