So, to answer your question...
It was an accident made to look like it might be on purpose. At least that was the intent.
This one was my second book ever (Living the Good Death was the first) and also my first series, so looking back there are a LOT of things I'd fix/clarify with 20/20 hindsight. That said, more does come to light in the later books (though you have an almost literal mountain of books at this point, so I won't take it personally if you skip them ;) -- I do not envy your move!)
