You shouldn't feel stupid. It took me a long time before I knew. I re-read the ending many times and even went to Wikipedia to finally understand what happened.
You're not stupid at all. I think the hole point the author was trying to make is that abuse is not out in the open. he wanted the reader to read between the lines in the book and in life. I think you should reread the book and look for the subtle clues, for practice. Hopefully that will help you in the future.
