It's been awhile since I read so a lot of the plot and the character names are escaping me. But there was one character that several times her actions were described as "nails biting into her palms" when anxious. That author is Lauren Willig. I like her writing but she uses this saying far too much. It's very annoying over the course of a novel. I also believe Karen White's writing is distinctive and she often writes about Southern women. It was evident to me she authored the character who was married to the man who owned the art. Again.. Sorry I can't even remember character names but maybe that helps. This then leaves the other character as Beatriz Williams. I had only read short stories from her before this book so she was the least obvious to me. (less)

Also, I'm assuming the mouth sore is oral herpes, which was generally associated with prostitutes or the "unclean". Makes for a contrast that such a dignified woman would have that type of disease.
Perhaps its possibly a reference to her past, or maybe her association with the arts (and artists). At the turn of the century there was a budding lesbian society, specifically among the upper middle class, that was tied to some romanticism and the rise of women getting the right to vote and more freedom.
