Especially beginning in Volume 5, the violence becomes strongly R-rated. Plus, several characters along the way have disturbing deformities with disturbing, violent backstories that are revealed.

some of the middle grade kids at the school I work at love this manga. A 6th grader was the one who recommended it to me.
It's not for every kid, it definitely is gory, but they get so much violence from other shows and games and manga too. The story is one of hope and perseverance despite the gory and terrible odds.
A blanket statement that this isn't for middle-grade kids at all is incorrect.
7th/8th grade is who I would start to recommend it to, but I wouldn't stop a kid who wants to read it from checking it out. (less)
