I'm not sure if it's supposed to be a manifestation of Ben's grief, but to him, it's real. (Also, when you get to the end, you see that supernatural things are real in this book.) As the reader, you just have to accept it as the reality of this kid's world, just like you have to accept magic as being real when you read a fantasy book.
