In the book, the phrase "pagan babies" refers to babies born into families that worshipped African gods.
As the character Terry says, there are few pagan babies these days due to the high number of conversions to Christianity. Impressionistically: A generation or two ago in Catholic education and missionary circles one might hear about baptizing pagan babies in Africa, as in raising money to support priests baptizing pagan babies in Africa. One rarely hears the phrase nowadays. I believe the author Elmore Leonard is dating himself by putting the phrase into the female character's mouth. But as someone who went to a Catholic grammar school that raised money for the missions, I got the reference.(less)
