We Hate Those Who Resemble Us Most: Narcissism of Small Differences

It is common to believe that the more marked the differences between newcomers and citizens, the more pronounced the resultant racism. After all, white Frenchmen, Americans, and Dutch hotheads attack black folks. The self-proclaimed liberal white often harbour averse racism (unconscious racist attitudes). But, this is only half the truth. The ugliest manifestations of racism (up to genocide) are reserved to immigrants who look, act, and talk like us. The more they try to emulate and imitate us, the harder they attempt to belong, the more ferocious our rejection of them.
Freud coined the phrase "narcissism of small differences" in a paper titled "The Taboo of Virginity" that he published in 1917. Referring to earlier work by British anthropologist Ernest Crawley, he said that we reserve our most virulent emotions – aggression, hatred, envy – towards those who resemble us the most. We feel threatened not by the Other with whom we have little in common – but by the "nearly-we", who mirror and reflect us.
The "nearly-he" imperils the narcissist's selfhood and challenges his uniqueness, perfection, and superiority – the fundaments of the narcissist's sense of self-worth. It provokes in him primitive narcissistic defences and leads him to adopt desperate measures to protect, preserve, and restore his balance. I call it the Gulliver Array of Defence Mechanisms.
The very existence of the "nearly-he" constitutes a narcissistic injury. The narcissist feels humiliated, shamed, and embarrassed not to be special after all – and he reacts with envy and aggression towards this source of frustration.
In doing so, he resorts to splitting, projection, and Projective Identification. He attributes to other people personal traits that he dislikes in himself and he forces them to behave in conformity with his expectations. In other words, the narcissist sees in others those parts of himself that he cannot countenance and deny. He forces people around him to become him and to reflect his shameful behaviours, hidden fears, and forbidden wishes.
But how does the narcissist avoid the realisation that what he loudly decries and derides is actually part of him? By exaggerating, or even dreaming up and creatively inventing, differences between his qualities and conduct and other people's. The more hostile he becomes towards the "nearly-he", the easier it is to distinguish himself from "the Other".
To maintain this self-differentiating aggression, the narcissist stokes the fires of hostility by obsessively and vengefully nurturing grudges and hurts (some of them imagined). He dwells on injustice and pain inflicted on him by these stereotypically "bad or unworthy" people. He devalues and dehumanises them and plots revenge to achieve closure. In the process, he indulges in grandiose fantasies, aimed to boost his feelings of omnipotence and magical immunity.
In the process of acquiring an adversary, the narcissist blocks out information that threatens to undermine his emerging self-perception as righteous and offended. He begins to base his whole identity on the brewing conflict which is by now a major preoccupation and a defining or even all-pervasive dimension of his existence.
Very much the same dynamic applies to coping with major differences between the narcissist and others. He emphasises the large disparities while transforming even the most minor ones into decisive and unbridgeable.
Deep inside, the narcissist is continuously subject to a gnawing suspicion that his self-perception as omnipotent, omniscient, and irresistible is flawed, confabulated, and unrealistic. When criticised, the narcissist actually agrees with the critic. In other words, there are only minor differences between the narcissist and his detractors. But this threatens the narcissist's internal cohesion. Hence the wild rage at any hint of disagreement, resistance, or debate.
Similarly, intimacy brings people closer together – it makes them more similar. There are only minor differences between intimate partners. The narcissist perceives this as a threat to his sense of uniqueness. He reacts by devaluing the source of his fears: the mate, spouse, lover, or partner. He re-establishes the boundaries and the distinctions that were removed by intimacy. Thus restored, he is emotionally ready to embark on another round of idealisation (the Approach-Avoidance Repetition Complex).
In a study titled "War and Relatedness", published by the National Bureau of Economic Research, the authors, Enrico Spolaore, Romain Wacziarg concluded:
"(T)he degree of genealogical relatedness between populations has a positive effect on their conflict propensities because more closely related populations, on average, tend to interact more and develop more disputes over sets of common issues ... (P)opulations that are genetically closer are more prone to go to war with each other, even after controlling for a wide set of measures of geographic distance and other factors that affect conflict, including measures of trade and democracy."

The Psychosexuality of Personality Disorders

Our sexual behavior expresses not only our psychosexual makeup but also the entirety of our personality. Sex is the one realm of conduct which involves the full gamut of emotions, cognitions, socialization, traits, heredity, and learned and acquired behaviors.
By observing one's sexual predilections and acts, the trained psychotherapist and diagnostician can learn a lot about the patient.
Inevitably, the sexuality of patients with personality disorders is thwarted and stunted. In the Paranoid Personality Disorder, sex is depersonalized and the sexual partner is dehumanized. The paranoid is besieged by persecutory delusions and equates intimacy with life-threatening vulnerability, a "breach in the defenses" as it were. the paranoid uses sex to reassure himself that he is still in control and to quell is anxiety.
The patient with Schizoid Personality Disorder is asexual. The schizoid is not interested in maintaining any kind of relationship and avoids interactions with others - including sexual encounters. He prefers solitude and solitary activities to any excitement sex can offer. The Schizotypal Personality Disorder and the Avoidant Personality Disorder have a similar effect on the patient but for different reasons: the schizotypal is acutely discomfited by intimacy and avoids close relationships in which his oddness and eccentricity will be revealed and, inevitably, derided or decried. The Avoidant remains aloof and a recluse in order to conceal her self-perceived shortcomings and flaws. The avoidant mortally fears rejection and criticism. The schizoid's asexuality is a result of indifference - the schizotypal's and avoidant's, the outcome of social anxiety.
Patients with the Histrionic Personality Disorder (mostly women) leverage their body, appearance, sex appeal, and sexuality to gain narcissistic supply (attention) and to secure attachment, however fleeting. Sex is used by histrionics to prop up their self-esteem and to regulate their labile sense of self-worth. Histrionics are, therefore, "inappropriately seductive" and have multiple sexual liaisons and partners.
The sexual behavior of histrionics is virtually indistinguishable from that of the somatic narcissist (patient with Narcissistic Personality Disorder) and the psychopath (patient with Antisocial Personality Disorder). But while the histrionic is overly-emotional, invested in intimacy, and self-dramatizing ("drama queen"), the somatic narcissist and the psychopath are cold and calculating.
The Somatic narcissist and the psychopath use their partners' bodies to masturbate with and their sexual conquests serve merely to prop up their wavering self-confidence (somatic narcissist) or to satisfy a physiological need (psychopath). The somatic narcissist and psychopath have no sexual playmates - only sexual playthings. Having conquered the target, they discard it, withdraw and move on heartlessly.
The cerebral narcissist is indistinguishable from the schizoid: he is asexual and prefers activities and interactions which emphasize his intelligence or intellectual achievements. Many cerebral narcissists are celibate even when married.
Patients with the Borderline Personality Disorder and the Dependent Personality Disorder both suffer from abandonment and separation anxieties and are clinging, demanding, and emotionally labile - but their sexual behavior is distinguishable. The borderline uses her sexuality to reward or punish her mate. The dependent uses it to "enslave" and condition her lover or spouse. The borderline withholds sex or offers it in accordance with the ups and downs of her tumultuous and vicissitudinal relationships. Narcissist Drives You Insane: Ambient Abuse and Gaslighting

Ambient abuse is the stealth, subtle, underground currents of maltreatment that sometimes go unnoticed even by the victims themselves, until it is too late. Ambient abuse penetrates and permeates everything – but is difficult to pinpoint and identify. It is ambiguous, atmospheric, diffuse. Hence its insidious and pernicious effects. It is by far the most dangerous kind of abuse there is.
It is the outcome of fear – fear of violence, fear of the unknown, fear of the unpredictable, the capricious, and the arbitrary. It is perpetrated by dropping subtle hints, by disorienting, by constant – and unnecessary – lying, by persistent doubting and demeaning, and by inspiring an air of unmitigated gloom and doom ("gaslighting").
Ambient abuse, therefore, is the fostering, propagation, and enhancement of an atmosphere of fear, intimidation, instability, unpredictability and irritation. There are no acts of traceable explicit abuse, nor any manipulative settings of control. Yet, the irksome feeling remains, a disagreeable foreboding, a premonition, a bad omen.
In the long term, such an environment erodes the victim's sense of self-worth and self-esteem. Self-confidence is shaken badly. Often, the victim adopts a paranoid or schizoid stance and thus renders himself or herself exposed even more to criticism and judgment. The roles are thus reversed: the victim is considered mentally deranged and the abuser – the suffering soul.
There are five categories of ambient abuse and they are often combined in the conduct of a single abuser:
I. Inducing Disorientation
The abuser causes the victim to lose faith in her ability to manage and to cope with the world and its demands. She no longer trusts her senses, her skills, her strengths, her friends, her family, and the predictability and benevolence of her environment.
The abuser subverts the target's focus by disagreeing with her way of perceiving the world, her judgment, the facts of her existence, by criticizing her incessantly – and by offering plausible but specious alternatives. By constantly lying, he blurs the line between reality and nightmare.
By recurrently disapproving of her choices and actions – the abuser shreds the victim's self-confidence and shatters her self-esteem. By reacting disproportionately to the slightest "mistake" – he intimidates her to the point of paralysis.
II. Incapacitating
The abuser gradually and surreptitiously takes over functions and chores previously adequately and skilfully performed by the victim. The prey finds itself isolated from the outer world, a hostage to the goodwill – or, more often, ill-will – of her captor. She is crippled by his encroachment and by the inexorable dissolution of her boundaries and ends up totally dependent on her tormentor's whims and desires, plans and stratagems.Moreover, the abuser engineers impossible, dangerous, unpredictable, unprecedented, or highly specific situations in which he is sorely needed. The abuser makes sure that his knowledge, his skills, his connections, or his traits are the only ones applicable and the most useful in the situations that he, himself, wrought. The abuser generates his own indispensability.
III. Shared Psychosis (folie a deux)
The abuser creates a fantasy world, inhabited by the victim and himself, and besieged by imaginary enemies. He allocates to the abused the role of defending this invented and unreal Universe. She must swear to secrecy, stand by her abuser no matter what, lie, fight, pretend, obfuscate and do whatever else it takes to preserve this oasis of inanity.
Her membership in the abuser's "kingdom" is cast as a privilege and a prize. But it is not to be taken for granted. She has to work hard to earn her continued affiliation. She is constantly being tested and evaluated. Inevitably, this interminable stress reduces the victim's resistance and her ability to "see straight".
IV. Abuse of Information
From the first moments of an encounter with another person, the abuser is on the prowl. He collects information. The more he knows about his potential victim – the better able he is to coerce, manipulate, charm, extort or convert it "to the cause". The abuser does not hesitate to misuse the information he gleans, regardless of its intimate nature or the circumstances in which he obtained it. This is a powerful tool in his armory.
V. Control by Proxy
If all else fails, the abuser recruits friends, colleagues, mates, family members, the authorities, institutions, neighbours, the media, teachers – in short, third parties – to do his bidding. He uses them to cajole, coerce, threaten, stalk, offer, retreat, tempt, convince, harass, communicate and otherwise manipulate his target. He controls these unaware instruments exactly as he plans to control his ultimate prey. He employs the same mechanisms and devices. And he dumps his props unceremoniously when the job is done.
Another form of control by proxy is to engineer situations in which abuse is inflicted upon another person. Such carefully crafted scenarios of embarrassment and humiliation provoke social sanctions (condemnation, opprobrium, or even physical punishment) against the victim. Contrary to misinformation spread by "experts" online, covert narcissists are not cunning and manipulative. Classic narcissists are: they often disguise their true nature effectively, knowingly, and intentionally. They are persistent actors with great thespian skills. Not so the covert narcissist: he suppresses his true nature because he lacks the confidence to assert it. His is not a premeditated choice: can't help but shy away. The covert narcissist is his own worst critic.
Inverted narcissists are covert narcissists. They are self-centred, sensitive, vulnerable, and defensive, or hostile, and paranoid. They harbour grandiose fantasies and have a strong sense of entitlement. They tend to exploit other, albeit stealthily and subtly. Covert narcissists are aware of their innate limitations and shortcomings and, therefore, constantly fret and stress over their inability to fulfil their unrealistic dreams and expectations. They avoid recognition, competition, and the limelight for fear of being exposed as frauds or failures. They are ostentatiously modest.
Covert narcissists often feel guilty over and ashamed of their socially-impermissible aggressive urges and desires. Consequently, they are shy and unassertive and intensely self-critical (perfectionist). This inner conflict between an overwhelming sense of worthlessness and a grandiose False Self results in mood and anxiety disorders. They team up with classic narcissists (see below), but, in secret, resent and envy them.

To "qualify" as an inverted narcissist, you must CRAVE to be in a relationship with a narcissist, regardless of any abuse inflicted on you by him/her. You must ACTIVELY seek relationships with narcissists and ONLY with narcissists, no matter what your (bitter and traumatic) past experience has been. You must feel EMPTY and UNHAPPY in relationships with ANY OTHER kind of person. How the False Self Uses and Abuses the True Self

Once formed and functioning, the False Self stifles the growth of the True Self and paralyses it. Henceforth, the ossified True Self is virtually non-existent and plays no role (active or passive) in the conscious life of the narcissist. It is difficult to "resuscitate" it, even with psychotherapy. The False Self sometimes parades the child-like, vulnerable, needy, and innocent True Self in order to capture, manipulate, and attract empathic sources of narcissistic supply. When supply is low, the False Self is emaciated and dilapidated. It is unable to contain and repress the True Self which then emerges as a petulant, self-destructive, spoiled, and codependent entity. But the True Self’s moments in the sun are very brief and, usually, inconsequential.
The False Self replaces the narcissist’s True Self and is intended to shield him from hurt and narcissistic injury by self-imputing omnipotence, omniscience, and omnipresence (godlike attributes).
The narcissist pretends that his False Self is real and demands that others affirm this confabulation.
The False Self re-interprets information in a flattering or socially-acceptable light and also imitates and emulates normal emotions and empathy.
Question:
Why does the narcissist conjure up another Self? Why not simply transform his True Self into a False one?
Answer:
We often marvel at the discrepancy between the private and public lives of our idols: celebrities, statesmen, stars, writers, and other accomplished figures. It is as though they have two personalities, two selves: the "true" one which they reserve for their nearest and dearest and the "fake" or "false" or "concocted" one which they flaunt in public.
In contrast, the narcissist has no private life, no true self, no domain reserved exclusively for his nearest and dearest. His life is a spectacle, with free access to all, constantly on display, garnering narcissistic supply from his audience. In the theatre that is the narcissist's life, the actor is irrelevant. Only the show goes on.
This substitution is not only a question of alienation, as Horney observed. She said that because the Idealised (=False) Self sets impossible goals to the narcissist, the results are frustration and self hate which grow with every setback or failure. But the constant sadistic judgement, the self-berating, the suicidal ideation emanate from the narcissist's idealised, sadistic, Superego regardless of the existence or functioning of a False Self.There is no conflict between the True Self and the False Self.
First, the True Self is much too weak to do battle with the overbearing False. Second, the False Self is adaptive (though maladaptive). It helps the True Self to cope with the world. Without the False Self, the True Self would be subjected to so much hurt that it will disintegrate. This happens to narcissists who go through a life crisis: their False Ego becomes dysfunctional and they experience a harrowing feeling of annulment.
The False Self has many functions. The two most important are:
1. It serves as a decoy, it "attracts the fire". It is a proxy for the True Self. It is tough as nails and can absorb any amount of pain, hurt and negative emotions. By inventing it, the child develops immunity to the indifference, manipulation, sadism, smothering, or exploitation – in short: to the abuse – inflicted on him by his parents (or by other Primary Objects in his life). It is a cloak, protecting him, rendering him invisible and omnipotent at the same time.
2. The False Self is misrepresented by the narcissist as his True Self. The narcissist is saying, in effect: "I am not who you think I am. I am someone else. I am this (False) Self. Therefore, I deserve a better, painless, more considerate treatment." The False Self, thus, is a contraption intended to alter other people's behaviour and attitude towards the narcissist.
These roles are crucial to survival and to the proper psychological functioning of the narcissist. The False Self is by far more important to the narcissist than his dilapidated, dysfunctional, True Self.
The two Selves are not part of a continuum, as the neo-Freudians postulated. Healthy people do not have a False Self which differs from its pathological equivalent in that it is more realistic and closer to the True Self.
It is true that even healthy people have a mask [Guffman], or a persona [Jung] which they consciously present to the world. But these are a far cry from the False Self, which is mostly subconscious, depends on outside feedback, and is compulsive.
The False Self is an adaptive reaction to pathological circumstances. But its dynamics make it predominate, devour the psyche and prey upon the True Self. Thus, it prevents the efficient, flexible functioning of the personality as a whole.
That the narcissist possesses a prominent False Self as well as a suppressed and dilapidated True Self is common knowledge. Yet, how intertwined and inseparable are these two? Do they interact? How do they influence each other? And what behaviours can be attributed squarely to one or the other of these protagonists? Moreover, does the False Self assume traits and attributes of the True Self in order to deceive the world?Let's start by referring to an oft-occurring question:
Why are narcissists not prone to suicide?
The simple answer is that they died a long time ago. Narcissists are the true zombies of the world.
Many scholars and therapists tried to grapple with the void at the core of the narcissist. The common view is that the remnants of the True Self are so ossified, shredded, cowed into submission and repressed – that, for all practical purposes, the True Self is dysfunctional and useless. In treating the narcissist, the therapist often tries to construct and nurture a completely new healthy self, rather than build upon the distorted wreckage strewn across the narcissist's psyche.
But what of the rare glimpses of True Self oft reported by those who interact with the narcissist?
Pathological narcissism is frequently comorbid with other disorders. The narcissistic spectrum is made up of gradations and shades of narcissism. Narcissistic traits or style or even personality (overlay) often attach to other disorders (co-morbidity). A person may well appear to be a full-fledged narcissist – may well appear to be suffering from the Narcissistic Personality Disorder (NPD) - but is not, in the strict, psychiatric, sense of the word. In such people, the True Self is still there and is sometimes observable.
In a full-fledged narcissist, the False Self imitates the True Self.
To do so artfully, it deploys two mechanisms:
Re-Interpretation
It causes the narcissist to re-interpret certain emotions and reactions in a flattering, socially acceptable, light. The narcissist may, for instance, interpret fear as compassion. If the narcissist hurts someone he fears (e.g., an authority figure), he may feel bad afterwards and interpret his discomfort as empathy and compassion. To be afraid is humiliating – to be compassionate is commendable and earns the narcissist social commendation and understanding (narcissistic supply).
Emulation
The narcissist is possessed of an uncanny ability to psychologically penetrate others. Often, this gift is abused and put at the service of the narcissist's control freakery and sadism. The narcissist uses it liberally to annihilate the natural defences of his victims by faking empathy.
This capacity is coupled with the narcissist's eerie ability to imitate emotions and their attendant behaviours (affect). The narcissist possesses "emotional resonance tables". He keeps records of every action and reaction, every utterance and consequence, every datum provided by others regarding their state of mind and emotional make-up. From these, he then constructs a set of formulas, which often result in impeccably accurate renditions of emotional behaviour. This can be enormously deceiving.

In the film “The Beaver”, the character played by Mel Gibson suffers from depression. He latches on to a tattered puppet in the shape of a beaver and communicates exclusively through it. The Beaver is everything its ostensible master isn’t: daring, creative, exuberant, omnipotent, and omniscient, gregarious, resourceful, charismatic, and charming; a good father, good CEO, and good company all around. In short: The Beaver is the reification of the protagonist’s False Self.
When his wife (Jodi Foster) confronts him, having exposed his confabulations and the need to let go of the contraption, The Beaver rages at her and asserts its superiority, invincibility, and brilliance. The depressive Walter – the True Self - is derided by The Beaver as a dysfunctional wreck, utterly dependent on the former’s ministrations and the interference it runs on his behalf. The film ends unrealistically with Walter mutilating his body – literally - in order to rid himself of the domineering and all-pervasive appendage. “Unrealistically” because narcissists never succeed in resuscitating their dilapidated and crushed True Self. The narcissist IS his False Self: in real life, Walter should have been devoured and consumed by The Beaver – but then we would not have had a typical, syrupy Happy Ending, now, would we?
Both the True Self and the False Self depend on the gaze of others. The False Self relies on adulation and attention – narcissistic supply – for the maintenance of the precarious, confabulated, fantastic, grandiose, and counterfactual narrative that is the narcissist’s persona, his public face. Without a constant flow of such high-quality input and feedback, without the adulating gaze, the narcissist crumbles like a house of ephemeral cards and resorts to a variety of dysfunctional, self-destructive, and self-defeating behaviors and defense mechanisms.
Similarly and equally, the True Self needs a loving gaze to sustain itself. Another person’s love serves two purposes: it confirms the existence of the True Self as a lovable object and thus lays the groundwork and facilitates the necessary and sufficient conditions for self-love; and it allows the True Self to perceive the existence of a “safe”, loving, and holding other. Such insight is at the very foundation of empathy.Do the False and True Selves ever fight it out, David vs. Goliath, Good vs. Evil, The Beaver vs. Walter?
Alas, they never do. The False Self is concocted by the narcissist to fend off hurt. It is a perfect, impenetrable, impermeable shield, a cocoon; it rewards the narcissist by flooding him with warm, fuzzy, exhilarating feelings; and it sustains the narcissist’s delusions and fantasies. The False Self is the narcissist’s dreams come true. In other words: as far as the narcissist is concerned, the False Self is adaptive and functional. The narcissist is emotionally invested in the False Self and he despises the True Self for having failed to cope with the exigencies and vicissitudes of the narcissist’s life.

He keeps records of every action and reaction, every utterance and consequence, every datum provided by others regarding their state of mind and emotional make-up. From these, he then constructs a set of formulas, which often result in impeccably accurate renditions of emotional behaviour. This can be enormously deceiving.

In the film “The Beaver”, the character played by Mel Gibson suffers from depression. He latches on to a tattered puppet in the shape of a beaver and communicates exclusively through it. The Beaver is everything its ostensible master isn’t: daring, creative, exuberant, omnipotent, and omniscient, gregarious, resourceful, charismatic, and charming; a good father, good CEO, and good company all around. In short: The Beaver is the reification of the protagonist’s False Self.
When his wife (Jodi Foster) confronts him, having exposed his confabulations and the need to let go of the contraption, The Beaver rages at her and asserts its superiority, invincibility, and brilliance. The depressive Walter – the True Self - is derided by The Beaver as a dysfunctional wreck, utterly dependent on the former’s ministrations and the interference it runs on his behalf. The film ends unrealistically with Walter mutilating his body – literally - in order to rid himself of the domineering and all-pervasive appendage. “Unrealistically” because narcissists never succeed in resuscitating their dilapidated and crushed True Self. The narcissist IS his False Self: in real life, Walter should have been devoured and consumed by The Beaver – but then we would not have had a typical, syrupy Happy Ending, now, would we?
Both the True Self and the False Self depend on the gaze of others. The False Self relies on adulation and attention – narcissistic supply – for the maintenance of the precarious, confabulated, fantastic, grandiose, and counterfactual narrative that is the narcissist’s persona, his public face. Without a constant flow of such high-quality input and feedback, without the adulating gaze, the narcissist crumbles like a house of ephemeral cards and resorts to a variety of dysfunctional, self-destructive, and self-defeating behaviors and defense mechanisms.
Similarly and equally, the True Self needs a loving gaze to sustain itself. Another person’s love serves two purposes: it confirms the existence of the True Self as a lovable object and thus lays the groundwork and facilitates the necessary and sufficient conditions for self-love; and it allows the True Self to perceive the existence of a “safe”, loving, and holding other. Such insight is at the very foundation of empathy.Do the False and True Selves ever fight it out, David vs. Goliath, Good vs. Evil, The Beaver vs. Walter?
Alas, they never do. The False Self is concocted by the narcissist to fend off hurt. It is a perfect, impenetrable, impermeable shield, a cocoon; it rewards the narcissist by flooding him with warm, fuzzy, exhilarating feelings; and it sustains the narcissist’s delusions and fantasies. The False Self is the narcissist’s dreams come true. In other words: as far as the narcissist is concerned, the False Self is adaptive and functional. The narcissist is emotionally invested in the False Self and he despises the True Self for having failed to cope with the exigencies and vicissitudes of the narcissist’s life.

Merely Anxious - or Full-fledged Narcissist?

Anxiety Disorders – and especially Generalised Anxiety Disorder (GAD) – are often misdiagnosed as Narcissistic Personality Disorder (NPD).
Anxiety is uncontrollable and excessive apprehension. Anxiety disorders usually come replete with obsessive thoughts, compulsive and ritualistic acts, restlessness, fatigue, irritability, difficulty concentrating, and somatic manifestations (such as an increased heart rate, sweating, or, in Panic Attacks, chest pains).
By definition, narcissists are anxious for social approval or attention (Narcissistic Supply). The narcissist cannot control this need and the attendant anxiety because he requires external feedback to regulate his labile sense of self-worth. This dependence makes most narcissists irritable. They fly into rages and have a very low threshold of frustration.
Like patients who suffer from Panic Attacks and Social Phobia (another anxiety disorder), narcissists are terrified of being embarrassed or criticised in public. Consequently, most narcissists fail to function well in various settings (social, occupational, romantic, etc.).
Many narcissists develop obsessions and compulsions. Like sufferers of GAD, narcissists are perfectionists and preoccupied with the quality of their performance and the level of their competence. As the Diagnostic and Statistical Manual (DSM-IV-TR, p. 473) puts it, GAD patients (especially children):
"… (A)re typically overzealous in seeking approval and require excessive reassurance about their performance and their other worries."
This could apply equally well to narcissists. Both classes of patients are paralysed by the fear of being judged as imperfect or lacking. Narcissists as well as patients with anxiety disorders constantly fail to measure up to an inner, harsh, and sadistic critic and a grandiose, inflated self-image.
The narcissistic solution is to avoid comparison and competition altogether and to demand special treatment. The narcissist's sense of entitlement is incommensurate with the narcissist's true accomplishments. He withdraws from the rat race because he does not deem his opponents, colleagues, or peers worthy of his efforts.
As opposed to narcissists, patients with Anxiety Disorders are invested in their work and their profession. To be exact, they are over-invested. Their preoccupation with perfection is counter-productive and, ironically, renders them underachievers.
It is easy to mistake the presenting symptoms of certain anxiety disorders with pathological narcissism. Both types of patients are worried about social approbation and seek it actively. Both present a haughty or impervious facade to the world. Both are dysfunctional and weighed down by a history of personal failure on the job and in the family. But the narcissist is ego-syntonic: he is proud and happy of who he is. The anxious patient is distressed and is looking for help and a way out of his or her predicament. Hence the differential diagnosis.
