Sissy Carpenter nee Holmes is a 62 year-old widow trying to quit smoking. Her friend El suggests that she try hypnosis. The doctor puts her to sleep, telling her "You will not smoke again" and "I want to talk to that inner part of you." When Sissy wakes up, the room is full of police - the doctor was murdered while Sissy was sleeping!
Although Sissy no longer wants a cigarette, she now hears a voice in her head who identifies itself as Sherlock Holmes. At first she accuses her friend of playing a practical joke, then decides she needs to see a psychiatrist because of hearing the voice of a fictional character, but eventually decides this is an example of reincarnation since she may be a descendent of Sherlock Holmes.
The book is somewhat of a romp as Sissy/Sherlock and El drive around interviewing some of the doctor's other patients and neighbors in an attempt to solve the murder before one of them might be arrested.
I found this to be a fun story, but weak on characterization. Other than Sissy and El, I found it difficult to distinguish the characters. Interactions with the Sherlock voice were mostly along the lines of how things are different in 21st century Bethesda MD than they were in Victorian England, not much of Sherlock finding clues.
