Created by a physician and the author of Being Empowered for a Healthy A personal guide to taking control of your health while living with chronic conditions.
“The Heart Health Diary: A Comprehensive Health Journal for the Empowered Heart Disease Survivor” by Dr. Phoebe Chi is a guide for those suffering from adverse heart conditions.
It contains various health trackers (blood pressure log, sodium and fluid tracker, blood sugar log, symptom trackers), a medical planner section, health references, and a heart medication guide. I especially like the Heart Medication Guide. The author desires to help heart patients and those at risk of future heart ailments with the right information and motivation, so they can monitor and enhance their health.
This is literally just a journal to help people with ongoing heart issues write down everything and keep it all organized and together. There is no information or insight just a place to keep everything written down. It therefore doesn't work well in Kindle form, you really need the printed version to write on. I can see that this is a good tool for Drs to recommend to their patients to stay on top of their health.
More than a diary, this journal is helping me with the health concerns of two octogenarian. It features pages for medical information, hospital visits, medical tests and results, and a page with insurance info. The big items for me are the blood glucose pages and the medical appointments notes pages. This item is invaluable whether for yourself or family members.
Very Good Layout for a Journal The formatting is good, it's easy to read. There are helpful references in the back about different heart medications. A two year calendar, and also several checklists for health conditions to look for. There is also adequate blank space for notes along with small useful labels such as Date/Diagnosis and Medical Condition.
Able to pinpoint my blood pressure level by the use of scale provided.at least the writer should made more research on the medication quide to give the reader better information.
I actually had a big diet change recently, turns out I am allergic to corn, which is in everything. Since I now have a much more limited diet, I started researching all of its implications. This book was a big help.
