Hardbound. A wide variety of focal and systemic conditions have been ascribed to intracranial vascular compression of the lower cranial nerves and adjacent brainstem. These have included conditions as common as hypertension, hearing loss, tinnnitus and Bell's palsy, and others as apparently disparate as trigeminal neuralgia, hemifacial spasm and torticollis. It is likely that several hundred thousand patients have undergone surgery in order to relieve vascular compression for these disorders in the main trigeminal neuralgia and hemifacial spasm. For many conditions the published results are good. However, the procedures involve exploration of critical vascular structures within the posterior fossa and little, in some cases no, firm evidence has existed regarding the significance of the vascular relationships at which these procedures have been aimed.
