I have the 4th ed (trades in my 3rd Ed) and the biggest change is that it's been updated to include a whole host of primary care issues. I use this book all the time, found it was great when studying for comps and plan to use it when studying for boards. Great, concise, thorough guidelines. Always includes some alternative management options. Great for quick reference on management.
