Program design can sometimes be the simplest part of personal training or rehabilitative care, but it is usually the most important. The intensity, frequency, and modalities you use determine training someone for power, for stability, or injuring them in the process. Many trainers seem to go from session to session just thinking "I'm just going to throw workouts at them and hope it comes out alright," instead of creating a comprehensive plan where each session, and indeed each exercise, forms a synergistic plan to safely and successfully achieve the clients goals.
