I found my copy of this on Amazon for $0.35 plus shipping and handling. And it was worth every penny. The downside is this is a very dated edition from 2001 that has been revised several times already... and it shows! Much to my surprise, and despite every medic over the age of 40's assertion that things have changed a lot in medicine and EMS, I've found out that this is very much the case. Outdated as it is, the book is very simply organized and provides a wealth of pertinent information in an easily digestible format. Don't read this one, get the newer edition, but definitely give it a read if you're a new medic or one that is about to re-cert.
