Why did theo risk everything to go treat alicia ?
I am not sure that Theo's real intent was to treat Alicia, but rather an escalation of his obsession with her and because he feared that if she spoke, she would reveal who really killed her husband, Gabriel. I think he wanted to insure that she remained silent or otherwise manipulate the situation so that if she identified him as the killer, he could discredit her, claiming that it was some weird transference process in her therapy. He had to be close to her to control her as well as to enjoy his revenge fantasies about killing his wife's lover. Theo did not have an altruistic motive.
Theo did not go to treat Alicia; he went to see her face again after the horror he put her through. Plus, Theo had nothing at home for him. He wanted to watch Alicia struggle, and then was craving for her to realize it was him. He had an ulterior motive.
Somehow Theo saw himself reflected in Alicia since both had been victims of unfaithful partners. He perceives Alicia as a partner in his own misfortune, he believed that he did her a great favor by breaking into her house to "teach her the truth." I think that the fact that he risks everything lies in his need to understand his equal, Alicia; why doesn't she talk anymore? What is behind that action?
He often narrates his desire to kill the man his wife is sleeping with: this leads him to deny the idea that Alicia is "crazy", but instead seeks yet another explanation for why he decided to kill her boyfriend, because he had similar fantasies and doesn't think he's crazy, so Alice mustn't be either. Knowing Alicia's motives and denying her madness is a reflection of his need to deny the fact that he had a very similar desire, and thus denying the possibility that he is just as lost as Alicia.

I feel like Theo felt both guilty and curious about what happened. To me it seems a sthough he wanted to prove to himself that it wasn't his fault that Alicia killed her husband and that it was an underlying problem that he was yet to solve. Maybe he was simply intrigued by what triggered that amount of anger in Alicia, we know he wasn't mentally stable giving everything he did, so for him it could've been a sick game, a way out of his reality and past and a way into hers

I think perhaps Theo felt some guilt, but I also think that he was so obsessed with her.
he didnt want to treat alicia. he was trying to make sure his little secret didnt get out. her being diagnosed as mentally unstable and being put in a mental facility only magically played to his advantage
It was clear he had ulterior motives, and when Alicia found out Theo's identity, he did not hesitate to hurt her. It was clear that he was obsessed with Alicia, plus he needed reassurance his deed would never be exposed. Personally, I think he needs psychological help. He would be better fit the patient than the staff.

I feel Theo did felt a connection with Alicia. In his lost mind probably saw himself wanting to help her, but in reality he was trying to help himself. He needed to make sure she did not know him, or remember him but at the same time...he did all he could so she could speak again. I feel seeing her in that state just made him want to reach her the way he never was by his own family. Once Alicia started speaking, part of him was happy as a doctor, and because he probably thought she didn't remember him. He felt proud of an accomplishment, yet that same accomplishment came with a price...
His actual love life was a disaster and his own wife was lost in her mind, so finding Alicia was also a way for him to find fulfillment... unfortunately Alicia did remember him.

I think Theo was never there to treat Alicia, but he wanted to make sure she would not reveal who really killed her husband, Gabriel.
I think it's because he's obsessed with herand has feelings for her and he stopped loving his wife Kathy as you know he knows that he's the main reason she killed her husband and in the beginning of the book he wants to help her start speaking again as he explained to his boss so I think he wanted her to start talking again and healing and maybe see him as something more than just a therapist if she forgot about him being the main reason she's there at all
