SOLVED. Adult Drama - read in 2015 but probably published earlier - 20th Century American family, good Christians. Daughter becomes pregnant at 16 after a one-night-stand, mother sends her to France to hide pregnancy from the town.

She also assumes her pregnancy herself and raises the child as her own. The boy grows up thinking his biological mom is his sister. His other sister eventually has a daughter with whom he ends up falling in love. They start a relationship and she becomes pregnant but is quickly diagnosed with cancer after going to the doctor's for a toothache. She is told she will lose the baby if she undergoes treatment, but she wants the baby to live so she sacrifices herself and dies shortly after birth. The child is raised by its father, and I think they live on the road for a while (not 100% certain). At the end, the grandparents are on a trip to Rome and the very religious matriarch (who had sent her pregnant 16-year-old daughter to France) is robbed (?) and stabbed in an alley, and dies. I think she forgives her murderer as she goes...? I read the softcover and the cover was blue. Must've been 300-ish pages? Thanks!
