Yes I add them too after reading, but I need to check triggers before reading as there are lots of things that trigger me...
That’s why I add them in my reviews, as I understand how difficult it is to find them. I have to check them for every single book I read. :)

Yes it’s happened to me before too. I didn’t check warnings one time and that was the book that triggered me in the last 97% mark. 🤦‍♀️
