What exactly happened in the ending?

I recently finished "The Double" and am slightly confused on what happened in the ending. I'm not sure if it is just the way the translation was written but it was slightly confusing for me. Would someone be willing to explain it?
In the carriage doctor said to him that he'll get an apartment with light and maids, implying Jacob is going to psychiatric clinic. Golyadkin thought earlier that Ruthenspitz was kind of dumb but in this moment he sees doctor as some evil creature who takes him to the clinic. Although Jacob long ago have already felt this is going to happen.
