I wondered why Dorshaw wasn't given the contract on Millie to begin with since he had done the others. Dorshaw was a true psycho and a very smart one.

Dorshaw killed those who were mutilated, but some of the other victims weren't. It seemed to me that Dashworth might have been afraid of Dorshaw and didn't want to give him more "work".
In answer to the question, something always seemed to get in the way of Christopher killing him. I expect the author wanted to keep him around to heighten the suspense.
It felt like a combination of both. Argent was clearly distracted but others kept interfering at the most inopportune moments. I also agree that the author figured it would spice up the intrigue.
I don't think Argent underestimated Dorshaw at all; I think he knew there was a large possibility that he would escape from the police. Maybe his estimation was skewed by never considering that Millie's son would go so far to protect his mother, and vice versa, when he was out on the hunt [while assuming that they were safely under the underworld king's protection all that time.]
