I read this in the early 2000s - definitely before 2008. It seemed like an older book to me, but I can't be sure. I don't remember much about it. I think there was a scene where they made love in nature - possibly on a beach. And there was a scene where a character was chased, maybe through a jungle? I mainly remember that they were lovers, and then somehow woke up as each other's races.

Brazil? There's such switch and they spent some time hungry in a jungle or a beach (there was lovemaking, I remember that :) ), chased or running from something, I can't remember.
Yup, that is definitely it. I recognize the 90s cover and some of the very... descriptive writing mentioned in the reviews!
