In case she's misremembering details, I'll throw this one out: Silent to the Bone. The little sister was injured, not kidnapped, but the boy has become mute as a result and his friend has to figure out how to communicate with him to find out what happened.

"My friend is looking for a book she read in middle school (around 2001-2004) about a boy who was supposed to take care of his little sister but then was accused of abuse, once it was all said and done the boy becomes mute or refuses to speak.

She said the book might have had an orange cover."
