SOLVED. A fantasy book about an island? Spoiler ahead. [s]

I read this book in early middle school so around 2008ish? From what I remember it was about an island that had a large ring then in the middle of the island was an island much higher for the higher class? The outer ring was lower class? I don't remember much aside from the islands and the fact that they ate some meat which I think was important? I'm grasping at straws here I'm sorry but I know this book exists. Probably. SPOILER - I know at one point the character is falling and shoves his arm into a hole in the wall and straight breaks his arm.

Apologies for the late reply! I believe it was YA? The protag would've been a teenager I believe. It's so bothersome how little I remember but I see what the world looked like.
