Ya fiction about girl with cancer she starts at new school and meets a boy

I read this book a few years back and it was about a girl who had cancer and was starting fresh at a new school and she meets a popular boy who plays footy they fall in love the popular mean girl gives her a hard time and even slashes the guys tyres when they are on a date she starts to get sick again and I think needs a kidney transplant and the boy secretly donates his even though it means giving up on football
