Replaced the placeholder cover on the Kindle edition, merged a duplicate and updated the page count.

There are 2 kindle entries. One without a bookcover and one with. The one with a bookcover doesn't have a page count. Amazon is showing 264 pages. (The one without a bookcover shows 266 so it'd be an update anyways).
