Part of the confusion is that coffee table books such as these often had many hands involved. Here is the list from the title page of the first edition (1958):
By the Editors of American Heritage, The Magazine of History
Editor in Charge: Richard M. Ketchum
Narrative by Bruce Lancaster
with a chapter by J. H. Plumb
Introduction by Bruce Catton.
The first, 1958, edition was published by American Heritage Publishing Co., distributed by Simon & Schuster. It has 384 pages. One of the editions listed in GR is 1958 by Forbes Custom Publishing, but I'm fairly sure that can't be correct, since Forbes would have done a reprint.

Can someone straighten this all out? I'd like to see Bruce Lancaster credited as primary author, with Ketchum listed after him as editor in charge, then Plumb, then Catton. But whatever you librarians feel is best is fine.
