How to add cover, if not using Amazon?

Hello, am a bit confused, many new terms to try to understand. But can someone tell me how to add my cover if not using Amazon? Putting the book page below (don't even know if that's what this page is called!)

I added the cover which I got from your publisher site. I also put the ISBNs in their appropriate fields. The title shouldn't include the author/ISBN, so removed those too.
