How does a person get a training score of 11?

Without making the gamemakers mad,how does one get a score of 11?The Careers have been training since they were young kids and the best they can manage is 10.

A score of 11 over 10 is meant for more attention to be drawn to the tribute, I believe. Also, in catching fire (view spoiler)[Katniss and Peeta both get 11's for the purpose of the other tributes to be out to get them. (hide spoiler)]
Whereas a score of 10 just means they are real good, like a career.

You need to really pi** off the judges so that you're a target.
