I have to laugh...there is a whole group on goodreads that talks ONLY about who should Sookie Stackhouse end up with.
Could Romeo and Juliet be counted? ^_^For me, it's from Jonathan Stroud's Bartimaeus Trilogy: The Amulet of Samarkand, The Golem's Eye, Ptolemy's Gate. Nathaniel and Kitty should have ended together (death or alive, I don't care, just together!) Instead, it ended in a very painful way. Noble and heroic and beautiful *whatever* ... but painful.
I was heartbroken and left sobbing when Gemma Doyle and Kartik did not end up together in Libba Bray's "The Far Sweet Thing". I spent the entire series hoping that would be the case and was heartbroken when it didn't happen.
Well, this is an old one, but I was very disappointed that Walter Hartwright ended up with Laura Fairlie rather than her half-sister, Marian Halcombe, in The Woman in White. Laura was an incarnation of the basic pretty and helpless victorian heroine, while Marian was intelligent, witty and resourceful. Of course, she was also unattractive so that meant she was doomed to maiden-aunthood by any victorian novelist.

The first two that came to mind were Garrett and Theresa in Message in a Bottle and John and Savannah in Dear John. I'm sure if I thought about it longer, I could come up with some that weren't at the hands of Nicholas Sparks!
