They get ARCs provided (usually) by the publisher - sometimes you need to have a large following and a review blog to qualify (a requirement of many publishers on Netgalley). Sometimes the author themselves give them away in exchange for reviews. Or because they work at a bookstore that gets early copies. There's also contests, such as the one currently open on GR.
I don't know how people who got this early (if any did) specifically got it, but I'd assume it was one of the ways I mentioned above.
