It most likely is because of the usage of so many names. Most warriors share similar names for instance Cloudstar's story mentions a Maplestar and I remember Mapleshade and confuse them. So likely the authors have trouble keeping track of the names when most have similar names being used.

I think it might be because Erin Hunter has a hard time keeping track of all those names.
