Hmm.... this is a good answer, but then how come he still displayed the signs of the infection/worm? He didn't have any pain when he was cutting into himself
I believe he had a mental breakdown. He basically tortured himself by cutting into his own body, digging for the worms. He kept thinking he saw one, but it was always out of reach. By the time Shelley started cutting into him, he was too far gone mentally to feel the pain.
