Emerging conceptualizations of major emotional disorders emphasize their commonalities rather than their differences, including considerable overlap in disorder phenomenology, a common set of vulnerabilities to development of emotional disorders, and generalization of treatment response across

پروتکل یکپارچه، شیوهی درمانیای مشتق از رویکرد رفتاردرمانی شناختی CBT است که در سالهای اخیر برای درمان دایرهی گستردهای از اختلالات با نام «اختلالات هیجانی» تنظیم و عرضه گردیده است. در این مدالیته، اختلالات هیجانی سندرمهای متنوعی را شامل میشوند که به صورت کلی سه ویژگی اساسی دارند: فرد هیجانات ناخوشایند (نه لزوماً منفی) را با بسامد یا شدتی بالاتر از حد عادی تجربه میکند - واکنشِ به این هیجانات همراه با تنفر و بیزاری است؛ در واقع فرد حاضر به تحمل آنها نیست یا برداشتی ناکارآمد از آنها دارد - در اثر این بیزاری، فرد به رفتارهای مختلفی برای اجتناب از این هیجانات دست میزند که مجموعهی تمام این عوامل به تشدید این هیجانات در درازمدت منجر میشود. نکتهی جالب توجه این است که «هیجان» اینجا به عنوان پدیدهای چندمولفهای معرفی میشود: شامل فکرها، رفتارها و حالتهای بدنی متنوع، پس درمان نیز پله به پله از وجوه مختلف به موضوع ورود میکند تا در نهایت فرد گنجایش این را پیدا کند که هیجان آزارنده را درون ظرف روانیاش جای دهد؛ و «راه نیکوتر» را برگزیند.
عوامل مختلفی بالینگران را بر آن داشته تا برای این مجموعه از اختلالات، پروتکل درمانیای فراتشخیصی و یکپارچه طراحی کنند؛ مثلاً در نمونههای بالینی بسیاری از مراجعین این اختلالات را بصورت همبود نشان میدهند یا اینکه دادههای عصبشناختی حاکی از مدارهای مشترکی هستند که در این اختلالات فعال یا غیرفعالند. از جملهی این اختلالات انواع اضطرابها، افسردگی، دوقطبی، وسواسی-جبری، بدریختانگاری بدن، شخصیت مرزی، استرس پس از سانحه، اختلالات خوردن و بیخوابی است. اختلالاتی که شباهتشان به هم خیلی بیشتر از تفاوتشان است! در همهی آنها الگویی از وقوع هیجانات آزاردهنده وجود دارد که تعبیر و تفسیر بیزارگونه به همراه تلاش برای سرکوب، اجتناب یا فرار کردن از آنها موجب یک چرخهی معیوب میشود؛ و این هرچه بیشتر میخ عادتهای مخرب آن را در زندگی فرد میکوبد.
مثلاً فردی را در نظر بگیرید که واکنش بیزارگونهاش چنین است: "من نمیتونم تنهایی دووم بیارم/ از رها شدن خیلی میترسم" در نتیجه پیشدستانه رابطههایش را تمام میکند، مدام یارش را چک میکند یا اینکه در برابر جداییِ قریبالوقوع تهدید به خودکشی میکند. یا مثلاً فردی که فکر میکند: "من چقدر ضعیفم که دچار حملهی پنیک میشم/ قلبم وقتی تند بزنه یعنی آبروم قراره جلوی بقیه بره" در نتیجه از کافئین اجتناب میکند، فقط در ساعات غیرشلوغ بیرون میرود، حتماً کنار درب خروج مینشیند و حتی دارویی را همیشه با خود حمل میکند.
به نظرم آشنایی با این رویکرد میتواند حداقل در دستاندازهای زندگی شخصی بسیار روشنگر و آموزنده باشد.

Excellent, easy to read and easy to implement book for therapists. There is a workbook for clients. Very helpful for treatment of Anxiety Disorders and Depressive Disorders. Though the title can be a little off-putting if you don't like techno-jargon, the book itself is an easy and thorough read.

September 2, 2022Given that most of the books I read are academic, I should probably start putting them in here. Especially so I don't lose track of what I think. The premise of the UP is interesting. I still prefer more dynamic approaches, but I can also appreciate the structured nature of the approach. There are some clients who need the structure of homework and practice in order to be successful in therapy. I still hate mindfulness meditation, but given that it's a small part of the UP, I think I can manage.

AMAZING! Brilliant treatment modality. Gradually builds upon previous sessions so that clients can fully understand and implement skills they learn. Going to be using this throughout my career.
The therapist guide also provides you with great troubleshooting that comes up on therapy (and is spot on because some of those issues have arisen).

Really helpful book! It is organized to feel clinicians who want a strong core frame for treating anxiety and depressive disorders from a transdiagnostic empirical base to include values and mindfulness. Very useful.
