Once upon a time I reviewed nursing documentation as a key part of my job. If it isn't charted CORRECTLY, it didn't happen. You can be the world's best nurse and get destroyed in court. Charts are legal documents. Take the extra time and invest in your understanding of nursing as a craft. Every nurse needs to read this for their own practice... anyone wanting to go to leadership or administration is a liability unless they have this mastered.
