Almost exactly the same as the 3rd edition. The new edition does add new tests in the back of the book, and each subject has its own review test in the 3rd edition. The 2nd edition is a good, cheap alternative to the newer edition if you are strapped for cash. Get the newer edition if you can, but this will be sufficient for the HESI A2 as well.
