Don Howard

Don Howard is a physician-scientist, medical leader, and entrepreneur with more than three decades of experience spanning clinical medicine, scientific research, and healthcare innovation. He is the founder and former CEO of CellNetix Pathology and Laboratories, one of the largest private pathology firms in the U.S., where he led the development of advanced programs in molecular oncology and precision diagnostics. His work helped shape new models for delivering pathology services and cancer care. Don has also held academic and clinical leadership roles, including Clinical Associate Professor at the University of Washington and Chief of Pathology at Swedish Medical Center in Seattle.

He earned a medical degree from the Albany Medical College, a PhD in experimental pathology from the University of British Columbia and an undergraduate degree in philosophy from Boston University, bringing together scientific depth and philosophical perspective in his approach to medicine. Today, his focus is on writing philosophy and in particular metaphysics. He also collaborates closely with his wife and professional partner, Dr. Ioulia Howard, integrating individualized care, evidence-based longevity strategies, and emerging medical technologies. As co-authors, their work challenges conventional views on aging and explores practical ways to extend lifespan and improve how we age—clinically, personally, and socially.

I picked up Before Being because the question at its center has always haunted me: Why does the world exist? Why is there anything, anything at all? Many books mention that question, or take a stab at answering it, and then move quickly to avoid the hard questions this question requires. This book does not. It stays with the mystery and follows it as far as it can go.
What I appreciated most was the author’s treatment of “nothing.” Howard does not mean empty space, a vacuum, or a spiritual kind of emptiness. He means nothing in the strictest possible sense: no space, no time, no laws, no logic, no potential. That is not easy to think about, and I had to slow down in places, but that is also what gives the book its force.
This is serious reading without being cold. It begins in metaphysics, but the later chapters on consciousness, time, death, love, memory, and mystery make the argument feel deeply human. Those sections stayed with me most.
This is not light reading, especially early on, and readers should expect to be challenged. But I found the difficulty worthwhile, and even in the densest stretches I kept wanting to get back to it. Before Being does not pretend to solve the question of existence. Instead, it makes the question sharper, stranger, and harder to set aside. For a book on this subject, that is a difficult achievement.
If you’ve read Jim Holt’s Why Does the World Exist? or Sean Carroll’s The Big Picture, this is in the same territory, but Howard pushes the question in a more radical metaphysical direction in the book’s first half and yet still moving to the human lived dimension in the second. I’m looking forward to reading the next book in his trilogy, After Nothing.

I came to Before Being the way I come to most things: asking why. Why is there something rather than nothing? Why are we here to notice it? Why should any of it matter? These aren't idle questions for me—they're the ones I keep coming back to. So a book that opens with "Why is there something rather than nothing?" and then refuses to look away was always going to find a reader in me.
What I didn't expect was how carefully Don Howard would honor the question. Before Being is a rare and ambitious work: philosophically serious without becoming obscure, reflective without losing discipline, daring without pretending to settle what can't be settled. Howard follows the question with unusual persistence, moving from absolute nothingness through emergence, intention, consciousness, freedom, mortality, love, and meaning.
What impressed me most was the balance of rigor and humanity. The book offers no easy answers, but it never disappears into abstraction either. Instead it asks the reader to live with the mystery more honestly. For someone already drawn to these questions, reading it felt less like being instructed than accompanied. The prose is clear, often beautiful, and consistently thoughtful.
A fair warning: this isn't a casual read. It rewards patience and a real appetite for the questions it raises, and readers without some interest in philosophy, metaphysics, or the deeper "why" of existence may find it demanding. But that's less a flaw than a matter of fit. Howard isn't trying to make the subject easy—he's trying to take it seriously.
For anyone drawn to metaphysics, consciousness, theology, or the existential weight of being human, Before Being is a powerful and memorable book. It left me thinking long after I finished.

I am grateful that Kirkus Reviews gave Before Being: A Metaphysical Inquiry into Nothingness, Becoming, and the Mystery of Being Human its “Get It” verdict and described it as “a deeply academic and satisfying philosophical inquiry that tackles some of life’s biggest questions.”
That is what I hoped the book would do: take the question of absolute nothingness seriously—not as an abstract puzzle, but as a way of approaching existence, consciousness, and what it means to be human. These are difficult questions, and the book asks readers to slow down and think with care.
