Last Friday, 1 February, was the 10th anniversary of the Columbia shuttle disaster in which seven astronauts were killed when the NASA space shuttle burned up on re-entry into earth's atmosphere after a 16 day mission. The craft had been fatally compromised on take off by a falling piece of insulation foam that knocked a hole in the leading edge of the left wing, so that hot gases seeped into the wing as the craft attempted re-entry, causing it to explode.
