If people die early, before they reach retirement age, then they might not use Medicare and hence might be a net positive on the federal purse. If you use an accounting scheme by which a smoking death might actually be a positive thing, then you are doing it wrong. Which is to say that the main problems here are moral/ethical; the book doesn't touch those at all.
